How do I schedule an initial appointment?
Please email a request or call 277-4341 for an appointment. It would be helpful if you could fill out the online patient registration found on the “Forms” page of this website. Also, please print out your new patient forms, review and sign them. Please bring any insurance cards with you.
What insurances are accepted?

What if I don’t have insurance?
Payment is expected at the time of service. If extenuating circumstances or financial difficulties make payment difficult, we are willing to make special arrangements with you. Please contact our billing office.
What if a patient is under the age of 18?
A minor should be accompanied by their parent/guardian to their appointment, or they should have completed the “Consent of Treatment” form (see “Forms” page) signed by their parent/guardian.
